MRC Data System Account Request Forms:
Beginning June 2012, the Agency has a new data system account request process for both internal and external data systems and for employees and non-employees.
The system account creation process, has four steps - (1) the supervisor requests access for a user, (2) the assigned custodian for a system receives an email asking for approval of that access, (3) the system owner receives an email asking for approval, (4) once all approvals are granted, the user, supervisor, custodian and system owner receive an email saying the account has been created or documented; the final notification email includes terms of use and initial instructions.
For agency owned systems the user must also actively acknowledge terms of use on their first login; for non-agency owned systems the user, supervisor, custodian, or system owner may all have to be involved to set up the necessary access to the external data system.
Supervisors should initiate new account requests by logging in to the Agency portal:

Once logged in to the portal, supervisors will look under the System Access menu area and choose "Request for Employee" or "Request for Citizen", then follow instructions from there to choose the accounts needed.
If a system type or employee is not listed, contact MIS to have the listing modified.
